How to sign-up

To Register for Online Services, please bring the relevant form into reception with Photographic Identification so that we can register you for this service.

This form will allow you basic online access

    • Manage appointments
    • Request medication
    • Update demographic details
    • View your summary care record 

        Click here- Proxy Services Form

        An applicant can have Proxy access to another patient’s online services if;

          • The patient is aged 16 or over and lacks capacity to make decisions about their healthcare.
          • The patient is aged 0-11 years and the applicant can prove parental responsibility.
          • The patient has given their consent to allow the applicant proxy access. 

            For children between the ages of 11-16 we don't allow online services as per RCGP guidance.

                Winton Health Centre aims to provide a high quality, accessible, user friendly and progressive healthcare service for our patients utilising all available resources within our extensive multi-disciplinary team and from across the wider Primary Care Network.

                disability

                Our reception desk allows wheelchair-using patients to communicate directly with reception staff and our large car park area has disabled parking spaces and easy flat access into the building. There are also disabled toilet facilities.

                Improved access for patients

                We are working towards improving access to appointments for our patients, in particular at evenings and weekends. If you would like to be offered one of these appointments, please speak to a member of the team. These appointments may be at a different site e.g.  the Urgent Treatment Centre at Bournemouth Hospital.
